Out-of-Pocket clients: I can provide you with a superbill suitable for you to submit to your insurance. As each insurance plan is different, you will be responsible for contacting your insurance company and get the codes needed for the superbill. I will then add the codes to the superbill and send it to you for you to submit to your insurance. You agree to pay the full amount of the consult at the time of the visit (credit card or FSA). If the total fee for the visit is greater than the self-pay charge, I will not bill you for that balance. You agree that any fees paid by your insurance over and above the self-pay charge will be paid to me, either directly from your insurance or from you if your insurance sends you more than the self-pay fee.

Insurance-Covered Clients: Symphony LC is proud to partner with Wildflower and The Lactation Network (TLN) to offer lactation services covered by many major insurance plans. To verify your eligibility, please visit www.symphonylc.com and follow the appropriate links. Insurance approval does not guarantee continued coverage throughout the duration of your care. If your coverage changes or is discontinued, TLN or Wildflower will notify you directly. Should this occur, we will work with you to explore alternative options for ongoing support.

Cancellations: To cancel without rescheduling, a full refund will be issued if cancellation is more than 48hrs before the appointment. If the cancellation is after the 48hr window and you do not reschedule the appointment, a $45 cancellation fee will be collected.
